# Creating a Lead Form

Lead Forms give you a secure way to capture and manage inbound leads. Every submission automatically creates a contact and deal, so you can respond faster and grow your pipeline with less effort.

## Steps to Create a Lead Form

1. Go to your Account Settings > Leads > **Forms** section and click to add a **+New Form**.

2. A template form will appear which you can make adjustments to.

* **Internal Name**: This name is used only inside PaintScout to help you identify the form.
* **External Title**: This is the title and description displayed on the form for your leads to see.

3. Each Lead Form is made up of three sections:

<summary><strong>Project Details</strong></summary>

This section collects information about the customer’s painting project.

It includes a built-in question designed specifically for painting contractors that asks about the **type of project**. You can choose from pre-made icons or upload your own custom images to match your services and branding.

You can also add additional custom questions to better understand the scope of the project.

There is also an "Upload Images" functionality that you can toggle on or off to allow leads to upload helpful images and files.

<details>

<summary><strong>Contact Details</strong></summary>

This section captures the lead’s contact information. Any information collected here is saved directly to the contact, and in some cases, the deal. You can also rename some fields to better match your brand voice or the way your team works.

* **First name** and **last name** are required so PaintScout can automatically create a contact and deal
* **Email** and **phone number** correspond directly to the contact's information. These fields can be required or not.

To reduce the chances of having duplicate contacts in PaintScout, we recommend checking **Required** on the email field.

* **Address, city, state, zip/postal code** are not required fields but correspond to the deal and contact's address information. These can be toggled on or off to show on the lead form, and can be fully customized.
  * To ensure your new deals and contacts have the most accurate location information in PaintScout, make sure to show **all** address fields on your lead forms.
* **Lead source** corresponds directly to the new deal and contact's information. It is not a required field and can be toggled on or off on your lead form.

* **Preferred contact method** links to the contact's page. This can be customized on your lead form and can be toggled on or off.

<summary><strong>Appointment</strong></summary>

This section is for capturing information about the appointment, like what time of day is the lead available etc.

4. Re-order your lead form sections for extra customization

5. Once you have customized all your questions, make sure to click **Save Changes** at the top of the page.

6. Next, customize what happens after a lead submits the form.

Go to the **Settings** tab and choose one of the following options:

Display a confirmation message after submission. You can customize the text shown on this page.

<summary><strong>Redirect to external site</strong></summary>

Send leads to a specific URL after they submit the form, such as a thank-you page or scheduling link.

7. Next, set the **Acquisition Source** of the form.

PaintScout tracks **two different types of lead sources** on each deal:

* **Acquisition Source:** where the form was submitted, such as your website or an ad
* **Lead Source:** reflects the customer's answer to how they heard about your business, such as referral, Google search, or word of mouth. [Learn more](/support/operations/lead-forms/lead-source-vs-acquisition-source.md)

8. When your form is ready, you can share it via link or embed it on your website.
